You should immediately stop using the recalled books, pull the wings off the last page of the book and cut each wing into four pieces. Take a photograph of the 16 pieces, on or adjacent to the last page of the book, before disposing of the wings in a bin that is inaccessible to children. Removing the wings from the book removes this potential hazard. Complete the form below to register for your refund.

Submit one registration and one photograph for each recalled book. If you have more than one copy, complete a separate request for each book.
